Below we have provided the forms necessary to enroll as a client at ABC Resources. The process is quick and easy with only three required forms. You can download the forms marked with an asterisk (*) below and complete them at home. Just click on the link to start the download. After you have filled them out simply bring them to your first appointment. This way you can spend more time discussing issues with your therapist rather than completing paperwork. Additional forms have been provided below which people need from time to time. For example, if you need information or a report sent to your doctor, attorney, school, etc. you will need to complete a Release of Information Form. Other forms have been provided for informational purposes such as the Treatment Plan Form or the Coordination of Services Form which provide you with an idea of how treatment planning and coordination are conducted at ABC Resources.
